Relative Value Units (RVUs) are a measure used in healthcare to determine the value of services provided by medical professionals. RVUs quantify the amount of work, skill, and resources required to deliver a particular medical service or procedure. They are used to standardize and compare the relative value of different services.

Each RVU is assigned based on three components:

  • Work RVU: Reflects the effort and time the healthcare provider requires to perform the service.
  • Practice Expense RVU: Accounts for the costs associated with the overhead and resources needed to deliver the service, such as equipment and staff.
  • Malpractice RVU: Represents the cost associated with the risk of medical liability and malpractice insurance.

The RVU system helps establish service payment rates by multiplying the total RVUs by a conversion factor, translating these units into monetary values. This system is used in various billing and reimbursement processes, including those under Medicare.

How to calculate the conversion factor?

To calculate the conversion factor (CF) used to determine payment rates for healthcare services, follow these steps:

  • Determine Total RVUs: Calculate the total Relative Value Units (RVUs) for a specific service by summing the work RVU, practice expense RVU, and malpractice RVU.
  • Calculate Total Payment Amount: Identify the total payment amount or aggregate reimbursement for the services provided. The payer typically determines this amount or sets it by regulations.
  • Divide Total Payment by Total RVUs: To find the conversion factor, divide the total payment amount by the total RVUs. This provides the CF that translates RVUs into monetary values.

Example Calculation:

Suppose the total RVUs for a particular procedure are 5.0.  
If the total payment amount for this procedure is $500.

The conversion factor would be calculated as follows:
